With over 35 years as a registered nurse (RN) and extensive experience in hospice care, Denise Merillat has dedicated her life to supporting caregivers and their loved ones. Her journey through senior leadership roles and hands-on caregiving has provided her with deep empathy and insight into the challenges faced by caregivers. Denise combines her clinical expertise with holistic coaching to help caregivers reclaim their health, balance, and well-being. As a certified nurse coach, she’s passionate about empowering caregivers to set meaningful goals and restore their quality of life.

In 2023, 24.3% of U.S. adults (age 18 and older) experienced chronic pain. This is over 60 million people. 8.5% of U.S. adults experienced high-impact chronic pain, which significantly limits daily life or work activities. This equates to over 21 million people. New cases of chronic pain occur at a rate of 52.4 per 1,000 people per year, which is higher than the incidence rates of diabetes, depression, and high blood pressure. Chronic pain is highly persistent; approximately 61.4% of those who reported chronic pain in one year still reported it a year later.

Holistic chronic pain management, also known as Pain Reprocessing Therapy (PRT), focuses on retraining the brain to reduce neuroplastic pain by addressing psychological and emotional factors. Key methods include:
  • Understanding Neuroplastic Pain: Recognizing the brain’s role.
  • Pain Reprocessing Therapy (PRT): Techniques to rewire pain circuits through reattribution, cognitive reframing, somatic tracking, and exposure.
  • Addressing Emotions: Processing underlying stress, anxiety, and trauma.
  • Shifting Focus: Redirecting attention away from pain.
  • Building Safety: Reinterpreting sensations as non-threatening.
  • Mindfulness and Acceptance: Observing pain without judgment.
  • Gradual Activity: Slowly re-engaging in life.
  • Self-Compassion: Treating oneself with kindness.
Essentially, it’s about convincing the brain that the pain is not dangerous, thus reducing or eliminating it.

A significant study published in JAMA Psychiatry in 2022 by Ashar et al. compared PRT, Pain Reprocessing Therapy (PRT), to a placebo injection and usual care for chronic back pain. The results showed that:

  • 66% of patients in the PRT group were pain-free or nearly pain-free at the end of the 4-week treatment.
  • This is compared to 20% in the placebo group and 10% in the usual care group.

These significant reductions in pain were largely maintained at a 1-year follow-up, with 52% of the PRT group still reporting minimal pain. fMRI scans in the study indicated that PRT was associated with reductions in brain activity in pain-processing regions. This study provides strong evidence for the efficacy of PRT.
